Holy Name Medical Center in Teaneck, NJ is seeking an RN Care Transitions Coordinator to lead discharge planning and care coordination across the continuum of care. You will ensure safe, cost-effective delivery and collaborate with the interdisciplinary team to optimize patient outcomes.

Weekend and holiday coverage rotation is required as part of the schedule. Ideal candidates have 4–6 years in acute care with disease management/discharge planning experience, a valid RN license, and CCM

Qualifications

• RN license required upon hire
• CCM certification preferred
• 4-6 years of acute care experience
• 3-5 years of disease management and discharge planning experience
• Strong analytical and communication skills

Responsibilities

• Coordinate care transitions and discharge planning across the continuum
• Communicate with third party payors and care team
• Document patient records and ensure compliance with hospital policy
• Refer appropriate cases for social work intervention as needed
• Arrange post-acute referrals and transportation for discharge
• Support CMS requirements for observation patients and patient safety
