Job summary

Hackensack Meridian Health in New Jersey is seeking a Care Coordinator, Care Management to coordinate the clinical progression of patients' treatment and discharge plans. You will manage a designated caseload, collaborating with patients, families, and the multidisciplinary team to meet goals and arrange next level of care.

The role requires a BSN or pursuing one (or MSW), strong communication, problem-solving, and computer skills; NJ RN or SW licensure.

Qualifications

• BSN or BSN in progress and/or willing to acquire within 3 years of hire or transfer into the position; or Masters Degree in Social Work.
• Excellent verbal, written and presentation skills.
• Moderate to expert computer skills.

Responsibilities

• Coordinate, communicate, and facilitate the clinical progression of the patient's treatment and discharge plan.
• Oversee interfacility transitions and handoff between acute and post-acute services.
• Accountable for a designated patient caseload and meet treatment goals and expected length of stay.
