Job summary

BayCare's Winter Haven Hospital is hiring a Care Coordination Specialist in a PRN, As Needed role. You will collaborate with an interdisciplinary team to optimize clinical, quality, and financial outcomes, coordinating discharge planning and transitions of care to ensure appropriate post-acute resources.

A social work or healthcare-related degree with LPN licensure is preferred. Responsibilities include addressing social determinants of health, coordinating referrals, and supporting regulatory

Qualifications

• Bachelor's degree in social work, psychology, or healthcare related field.
• HS/GED with LPN license acceptable.
• Experience in medical social work or LPN preferred.

Responsibilities

• Collaborate with patients, nursing, physicians, and community partners to optimize outcomes.
• Coordinate comprehensive care and discharge planning to reduce readmissions.
• Address barriers related to social determinants of health in care plans.
• Coordinate transitions of care including referrals and post-acute services.
• Support compliance with regulatory requirements and value-based care initiatives.
