Job summary

Amedisys is seeking a Care Transitions Coordinator in NE San Antonio/Stone Oak. This role requires an RN, LPN, or another clinical license, plus outside sales/business development experience in healthcare.

Qualifications

• RN or LPN license required or other clinical license.
• Outside sales/business development experience in healthcare is required.
• Travel 50% or more within the assigned territory.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions and completes referral to admission process.
• Conducts on-site visits to assess patient needs and identify barriers to care.
• Keeps accurate records of patient interactions and referrals.
• Meets admissions and sales targets and documents daily calls in CRM.
• Attends community events to promote Amedisys.
