Job summary

Community Health Plan of Washington is seeking a Licensed Carceral Transitions of Care Manager (CT CM) to lead pre-release planning and short-term case management for members reentering the community from jail and corrections facilities. You will work with a multi-disciplinary team to assess health needs and coordinate services to achieve positive outcomes.

The role requires an active WA license (RN or LICSW/LASW), experience in behavioral health, and ability to collaborate with community

Qualifications

• Current, unrestricted license in WA as an RN, or LICSW/LASW.
• Background in behavioral health or working with diverse populations.
• Experience with community resources and Medicaid mental health/substance use services.
• Able to obtain an NPI and register with Washington State Medicaid ProviderOne.
• Valid WA driver’s license and access to reliable transportation.

Responsibilities

• Identify custody status and coordinate resources for justice-involved members during reentry.
• Collaborate with jails, DOC, Juvenile Rehabilitation and providers to assess health needs.
• Document case activity in CHPW care management system and maintain records.
• Schedule appointments and arrange transportation around release dates.
• Travel to carceral facilities and field work ~10% of the time.
