PIH Health is seeking an Access Coordinator to support safe, timely, and patient-centered transitions across the continuum of care in Los Angeles, CA. You will work with case managers, social workers, physicians, nurses, and the inpatient care team to facilitate throughput, discharge planning, transfers, and repatriation to home or lower/higher levels of care.

You will ensure EMTALA and payer compliance, coordinate information exchange, arrange post-discharge transportation, and provide bedside
