Qualifications

• High School Diploma or GED required.
• Licensed Vocational Nurse (LVN) and BLS certification required.
• Minimum three years of work experience in physician practice, healthcare, or care management.
• Bilingual language skills preferred: Spanish and Vietnamese.

Responsibilities

• Manage Care Transitions & Integration referrals with internal and external partners.
• Facilitate authorization procurement for third-party payers with clinical documentation.
• Coordinate care for complex patient needs and assist with outpatient procedures and placements.
• Maintain communication across continuum of care and ensure transportation for appointments.
• Document test results in the EMR and communicate with physicians and vendors.
