Job summary

Harris is seeking a Remote Chronic Care Coordinator to conduct telephonic encounters, develop care plans, and support patients with two or more chronic conditions. This Independent 1099 contractor role offers flexible hours and potential transition to full-time W2 with benefits, with bilingual incentive opportunities.

Ideal candidates are licensed RNs/LPNs, have EMR experience, and are proficient in Microsoft Office and HIPAA-compliant home workflows.

Qualifications

• Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
• Current active license to practice as an RN/LVN/LPN held in the state of MICHIGAN (NOT A COMPACT STATE) with no disciplinary actions noted
• A minimum of two years of clinical experience in Med/Surg, Case Management, and/or home health care
• Hands-on experience with Electronic Medical Records and understanding of Windows desktop and Office 365, in a HIPAA compliant home environment

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for multiple disease conditions through motivational coaching.
• Develop positive interactions with patients on behalf of our practices.
• Improve revenue by creating billable Chronic Care Management episodes for chronic conditions.
• Develop detailed care plans for doctors and patients for prevention and intervention purposes.
• Attend regularly scheduled meetings (Bi-Monthly Staff Meetings, monthly one-on-ones).
