Job summary

University Hospitals is seeking a Supervisor of Care Transitions to lead day-to-day operations across UH Lake West Medical Center, TriPoint Medical Center and Beachwood Medical Center. You will serve as the site content expert for all Care Transitions activities.

Collaborating with physicians, homecare ambassadors, UM, and post-acute partners, you will drive patient flow, reduce length of stay, and monitor NSOC and readmission prevention efforts.

Qualifications

• Experience leading care transitions or hospital discharge teams.
• Strong collaboration with physicians and post-acute partners.
• Familiarity with CMS conditions of participation.

Responsibilities

• Oversee day-to-day Care Transitions team operations.
• Collaborate with physicians, homecare ambassadors, post-acute facility liaisons, UM, and ACO/Population Health to ensure smooth patient transitions within UH network.
• Identify trends, barriers, and improvement strategies to decrease length of stay and improve patient experience.
• Monitor metrics to assess Care Transitions performance and determine next site of care.
• Provide guidance on complex psychosocial and discharge planning issues; escalate barriers with site leadership.
• Attend hospital committees and coordinate with local leadership on key metrics and team performance.
