Job summary

Frederick Health Hospital, Inc. is seeking a Case Management Assistant for Emergency Relief. You will work with the Integrated Care Management team to outreach to patients, assess transitions in care, and close care gaps related to quality measures.

The CMA will perform clinical and administrative tasks under supervision, collaborating with nursing and other departments, while supporting the health system's mission and compliance standards.

Qualifications

• AA degree with 3-5 years of clinical work experience preferred.
• Certified Medical Assistant or Certified Nursing Assistant certification preferred.
• Must have current CPR certification.

Responsibilities

• Outreach to attributed patients to close quality measure gaps and educate on health maintenance.
• Access and utilize multiple software systems to identify target patients and complete required documentation.
• Outreach to patients during care transitions to assess understanding, arrange follow-ups, and identify barriers to optimal health outcomes.
• Collaborate with the primary care team, other Frederick Health departments, and community partners.
• Identify Social Determinants of Health barriers and provide referrals to resources while coordinating with care managers.
• Engage with patients to foster trust and facilitate engagement with care management.
• Schedule appointments in the electronic health record and respond to messages and tasks from patients and the care team.
• Participate in staff meetings and other duties as assigned.
