Job summary

Aspire Physical Recovery Center at Hoover is seeking an experienced Case Manager to coordinate patient care, discharge planning, and successful transitions following rehabilitation.

This role demands collaboration with nursing, therapy, physicians, and community providers to move patients safely through the continuum of care. Strong communication, organizational skills, and a background in hospital case management or related healthcare areas are essential.

Qualifications

• Experience in hospital case management, nursing, therapy, rehabilitation, discharge planning, care coordination, or post-acute healthcare.
• Strong understanding of the patient transition and discharge-planning process.
• Excellent communication and interpersonal skills.
• Ability to manage multiple patients, priorities, and discharge plans simultaneously.
• Comfortable navigating complex patient and family situations.

Responsibilities

• Coordinate discharge planning from admission through transition.
• Meet with patients and families to identify discharge needs, concerns, and goals.
• Develop and coordinate safe, appropriate discharge plans.
• Arrange services, equipment, resources, and follow‑up needs for discharge.
• Work closely with nursing, therapy, physicians, and other interdisciplinary team members.
• Communicate with families, responsible parties, hospitals, community providers, and outside agencies.
• Identify barriers that could delay or complicate a successful discharge and help develop solutions.
• Participate in care conferences and interdisciplinary meetings.
• Coordinate transitions to home, assisted living, long‑term care, or other appropriate settings.
• Maintain accurate and timely documentation.
• Help patients and families understand available resources and next steps.
• Promote continuity of care throughout the transition process.
