Aspire Physical Recovery Center at Hoover is seeking an experienced Case Manager to coordinate patient care, discharge planning, and successful transitions following rehabilitation. This role connects patients, families, and the care team to ensure safe, effective progress through the continuum of care.

Ideal candidates bring hospital case management or related healthcare experience, strong communication, organization, and the ability to navigate complex discharge scenarios.

Qualifications

• Experience in hospital case management, care coordination, discharge planning, or related healthcare roles.
• Strong understanding of patient transitions and discharge planning processes.
• Ability to communicate effectively with patients, families, and interdisciplinary teams.

Responsibilities

• Coordinate discharge planning from admission through transition.
• Meet with patients and families to identify discharge needs and goals.
• Develop and coordinate safe discharge plans and arrange services/equipment.
• Work with nursing, therapy, physicians, and other team members.
• Maintain accurate documentation and communicate with external providers.
