Qualifications

• Bachelor’s degree in healthcare administration or related field.
• Current BLS/First Aid certification required.
• Valid driver's license and auto insurance, with personal automobile use.
• Minimum one year of clinical experience with frail or elderly population.
• Ability to work in a diverse, team-based environment and communicate with seniors and families.

Responsibilities

• Collaborates with RN case managers to ensure smooth transitions across care levels.
• Facilitates communication and coordinates care with members, families, and providers.
• Documents information in EMR per PACE policies.
• Schedules and coordinates specialty appointments and Day Center needs.
• Conducts regular visits to contracted RCFEs to facilitate care coordination.
