Job summary

Gary and Mary West PACE is seeking a Transportation Coordinator to ensure timely, safe access to the PACE center and medical appointments for participants. You will work within the Interdisciplinary Team to coordinate schedules with vendors and caregivers.

Qualifications

• High School diploma or GED required.
• 1 year scheduling/transport coordination in healthcare preferred.
• Experience with EMR systems and scheduling software.
• Proficient with Microsoft Office (Outlook, Word, Excel).
• Bilingual English/Spanish strongly preferred.
• Excellent organizational and communication skills.

Responsibilities

• Coordinate and maintain transportation for PACE participants to ensure timely access to services.
• Participate as an IDT member and communicate updates to families and staff.
• Schedule transportation with vendors, vendors, and internal teams; handle changes and issues.
• Document activities in the EMR and maintain participant confidentiality per HIPAA.
• Prepare transportation reports and support quality improvement initiatives.
