Qualifications

• 1-3 years of healthcare or patient services experience preferred.
• Knowledge of ICD-10 and CPT coding.

Responsibilities

• Assist with discharge planning by coordinating post-discharge services such as home health, transportation, or durable medical equipment.
• Facilitate insurance authorizations for post-acute care at skilled nursing facilities or inpatient rehab.
• Communicate with patients, families, providers, and external organizations to ensure timely and appropriate care transitions.
• Maintain accurate and organized records, perform data entry, and process necessary paperwork related to patient care coordination.
• Schedule follow-up appointments, transportation, or other services as directed by the Case Manager.
• Facilitate and track clinical reviews and authorization requests with payors by ensuring accurate and timely delivery of clinical documentation.
• Support quality improvement initiatives aimed at enhancing care coordination and patient satisfaction.
• Perform other duties as assigned.
• Maintain regular and reliable attendance.
• Comply with all policies and standards.
