Job summary

Compassus is seeking a Care Partner to coordinate safe, patient-centered transitions of care for hospitalized patients. The role focuses on discharge planning, post-acute referrals, and collaboration with physicians, case management, patients and families.

This in-person position requires bedside engagement across Torrance and Santa Monica Hospitals, with weekend coverage and one weekday in Torrance. You will educate patients and families on post-acute options and ensure timely handoffs to home

Qualifications

• Associate's degree required.
• Preferred: Bachelor's degree.
• Experience: none required; 2–3 years preferred in healthcare settings.

Responsibilities

• Evaluate patients for home-based and post-acute services based on clinical factors.
• Coordinate timely, safe discharge planning and transitions of care.
• Educate patients and families on post-acute options and care expectations.
• Maintain relationships with physicians, case management, nursing teams, and post-acute providers.
• Document care coordination activities in the medical record.
