Job summary

LCMC Health is seeking a Care Management Assistant to support the care management team with utilization review, intake, and post-discharge coordination in New Orleans. You will help ensure documentation and information flow to facilitate patient discharge and transition to post-acute care.

The role involves discharge planning activities, coordinating with post-acute providers, and transmitting care transition documents to physicians and payers.

Qualifications

• Combination of appropriate education and work experience.
• High school diploma/GED and 2 years of office/clerical experience in a healthcare setting.
• 4 years of experience in a healthcare setting is required in lieu of education.

Responsibilities

• Participate in discharge planning to ensure referrals to post-acute providers and timely discharges.
• Assist in identifying preferred post-acute care providers for discharge planning.
• Transmit and collect patient demographic and clinical information for post-acute care arrangements.
• Transmit transition of care documents to physicians and post-acute providers.
• Support payer communications and assist with denials and appeals processes.
