Job summary

Qualifications

• High school diploma or GED required with 2 years of office/clerical experience in a healthcare setting.
• 4 years of healthcare experience can substitute education requirements.

Responsibilities

• Assist in researching available post-acute care providers to meet discharge needs.
• Transmit referral information to post-acute providers and follow up for resource identification.
• Collect and transmit patient demographics and clinical information for post-acute arrangements.
• Transmit transition of care documents to the primary care physician and post-acute providers.
• Facilitate timely transmission of reviews to payers in coordination with the case management team.
• Communicate payer information to relevant stakeholders.
• Support concurrent insurance denials and appeals processes and provide denial information to stakeholders.
• Collaborate with departments to ensure accuracy of patient and payer information.
• Assist department leadership in data management.
• Maintain and update payer contact information in software systems.
• Validate and update vendor information.
• Schedule Weekend and Weekday PRN.
