VSHP Volunteer State Health Plan, Inc is seeking a Care Transition Case Manager to coordinate discharge planning and post-acute care for members with complex needs. You will perform face-to-face visits, collaborate with hospital teams on discharge planning, and connect members with resources to improve health outcomes and reduce gaps in care.

Ideal candidates will have strong clinical judgment, time management, and prioritization skills.

Qualifications

• RN license in Tennessee (or state of residence if multi-state license).
• 3 years clinical experience required.
• 5 years experience in the health care industry.
• For Select Community & Katie Beckett: 2 years experience in IDD.

Responsabilités

• Supporting utilization management for complex/non-routine cases as needed.
• Liaison between members, providers and internal/external customers.
• Overseeing highly complex cases to ensure interventions and efficient benefit use.
• Performing case management: assessment, planning, implementation, coordination, monitoring, outcomes and evaluation.
• Digital roles require effective communication via digital channels and tech support.
• Note: 18 month commitment before posting for other internal positions.
• Immunizations or related medical tests may be required.
• Job requires digital literacy assessment.
