You will manage a caseload, conduct visits, develop Person Centered Support Plans, and coordinate with private insurance and Medicaid to ensure services are appropriately covered. The ideal candidate holds an RN license in Tennessee (or a Nurse Licensure Compact state), has at least 3 years clinical experience and

Qualificações

• RN license in Tennessee or Nurse Licensure Compact state
• 3 years clinical experience
• 5 years experience in health care industry
• 2 years experience in IDD for Select Community & Katie Beckett

Responsabilidades

• Manage a caseload of multiple complex pediatric members' needs
• Conduct member visits including home visits and meet contract timelines
• Perform new member visits and ongoing assessments
• Develop and maintain Person Centered Support Plans
• Coordinate with private insurance (TPL) and Medicaid to ensure services are covered by the appropriate payor source
