Job summary

PIH Health is seeking an Access Coordinator to support safe, timely transitions of patients across the continuum of care. You will work with case managers, social workers, physicians, and the care team to coordinate throughput, discharge planning, transfers, and repatriation.

In this role you will ensure compliance with EMTALA, payer requirements, and hospital policies while serving as a consistent point of contact for patients and families.

Qualifications

• Coordinate patient transfers and discharge planning across care settings.
• Ensure EMTALA, payer requirements, and hospital policy compliance.
• Collaborate with case managers, social workers, physicians, nurses, and care team.

Responsibilities

• Facilitate patient throughput and transfers to higher/lower levels of care.
• Coordinate repatriation to home facilities and post-discharge follow-up.
• Maintain continuity of care through information exchange and bedside support.
