Job summary

Texas Health Resources, Inc. is seeking a part-time Care Transition Manager RN to join the Azle facility and support patient transitions across departments.

You will cross-train at the Alliance location in Fort Worth and provide coverage 24 hours per week with flexible shifts, including weekends and ER coverage as needed. Training spans four weeks, Monday to Friday, 7:30AM-4:00PM.

Qualifications

• Active nursing license valid in Texas.
• Experience in care transitions or patient discharge planning preferred.
• Ability to coordinate care across departments.

Responsibilities

• Support Med/Surg, ICU and ER departments as needed.
• Cross-train and cover at Texas Health Alliance locations.
• Facilitate patient transitions and coordinate care with teams.

• Will cross-train/cover at Texas Health Alliance at 10864 Texas Health Trail, Ft. Worth, TX 76244

Care Transition Management department

• Work hours: Part-time –24 hours/week, flexible coverage 7 days per week: Between Monday-Friday 7:30AM-4:00PM, on weekends Saturday & Sunday between 8:30AM-1:30PM, and ER coverage from 11:30AM-8:00PM.
• Does Require 4-weeks of training Monday to Friday 7:30AM-4:00PM
