Job summary

Texas Health Resources, Inc. is seeking a Care Transition Manager Social Worker at the Fort Worth facility to coordinate patient transitions and discharge planning.

LMSW or LCSW licensure is required, and the position is a part-time weekend role with 24 hours per week. The work schedule runs Saturday and Sunday from 7:00 AM to 7:30 PM, with up to five weeks of training Monday through Friday 7:30 AM–5:30 PM.

Qualifications

• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required.
• Part-time weekend position with 24 hours/week.
• Discharge planning and care transition coordination experience preferred.

Responsibilities

• Coordinate patient transitions and discharge planning within the hospital setting.
• Identify and address barriers to safe and timely discharge.
• Collaborate with care teams to optimize transitions and post-discharge planning.
