Endeavor Health is seeking a Transition Care Follow Up Specialist in Warrenville, IL to coordinate and complete post-discharge follow up orders. You will work both independently and as part of a team, following orders promptly, contacting staff for clarification, and engaging patients and families to ensure quality of care and reduce readmissions.

The role requires healthcare experience, Epic EMR proficiency, and strong communication.

Qualifications

• Six months of healthcare experience.
• Proficient in medical terminology.
• Strong organizational, written and oral communication skills.
• Able to handle multiple responsibilities, stress, and high workload volumes.
• Creative problem-solving skills.
• Demonstrates persistence in follow-up.

Responsibilities

• Provides age-appropriate care knowledge and skills.
• Contacts patients to schedule outpatient follow-up appointments, coordinating availability.
• Documents all patient encounters into EMR accurately.
• Contacts clinical staff for order clarification when needed.
• Performs general office duties including faxing sensitive information to providers.
• Participates in performance improvement activities.
• Duties as assigned.
