Job summary

Endeavor Health in Warrenville, IL is seeking a Medical Assistant for a full-time, 40-hour work week (Mon-Fri 8:30a–5:00p). The role pays hourly between $19.89 and $28.84, depending on experience and credentials.

In the Transitions of Care Navigation Program you will contact high-risk patients post-discharge, support initial assessments by phone, coordinate follow-ups, and help connect patients to resources and care options across inpatient to outpatient settings.

Qualifications

• Current certification from a national Medical Assistant body or obtain within 3 months of hire.
• BLS/CPR Certification for Healthcare Provider (AHA) required.
• Experience with clinical equipment and materials.

Responsibilities

• Telephonic outreach to post-discharge patients for initial assessment.
• Assist with connecting patients to resources.
• Coordinate post-hospitalization or ED follow-up appointments.
• Monitor tasks in portal for outreach and coordination of care.
• Support care teams with transitions of care activities.
