Job summary

PurposeCare is seeking a Care Transition Coordinator to grow our Home Health/Home Care presence within Supportive Living and community residences. You will generate referrals by building relationships with area hospitals, healthcare systems, case managers, and other community resources.

This role requires strong communication, the ability to work independently, and a commitment to HIPAA compliance. You will prepare weekly activity reports and help expand our referral network while representing

Qualifications

• Bachelor's degree preferred.
• One year of Home Health/Home Care marketing or case management experience.
• Valid driver’s license.
• Ability to work independently and meet deadlines.
• Excellent oral, written, and active listening skills.
• Uphold policies and HIPAA regulations.
• Ability to sit at a desk and listen effectively for long periods on the telephone.

Responsibilities

• Develop our Home Health/Home Care presence within Supportive Living and Community residences.

• Manage the end-to-end referral process including benefits verification and care coordination.
• Prepare weekly reports on marketing activities.

Job description

The Care Transition Coordinator is an experienced field marketer responsible for company growth. As a Care Transition Coordinator , you will be responsible for developing our Home Health /Home Care presence within Supportive Livings and Home and Community-based residences. Responsible for generating new referrals by building relationships with area hospitals, healthcare systems , Case Managers, and other community resources.

Essential Duties and Responsibilities:

• Participate in short and long-range planning to achieve company goals and objectives.
• Responsible for complete referral process, including identifying home health needs, verification of benefits, and coordination of care.
• Cultivate new and existing relationships resulting in company growth.
• Represent the agency in a professional, prepared, and caring manner.
• Prepare weekly reports on all marketing activities.
• Initiate and expand referral network.
• Comply with all aspects of the company's compliance and HIPPA regulations.
• Performs other duties as assigned.

Education and Experience :

• Bachelor's Degree Preferred
• One year of H ome H ealth /Home Care , marketing, or case management experience or a combination of each.
• Must possess a valid driver's license.
• This professional will uphold the policies and procedures established by Purpose Home Health. They must also demonstrate excellent oral, written, and active listening skills.
• Ability to work independently, maintain confidentiality, and meet assigned deadlines.
• Demonstrate sound judgment and good decision-making skills.
• Uphold integrity and fair-mindedness consistent with company standards, practices, policies, and procedures.
• Ability to organize and prioritize work.
• Establish good working relationships both internally and externally.

Qualifications and Requirements:

• Minimum 18 years old
• Ensure the schedule is completed in such a way as to retain all clients, hours, and caregivers
• Communicate staffing and availability needs to the appropriate supervisor
• Positive attitude, great communication skills, and ability to work independently
• A solid personal character with an empathetic personality
• Must be able to pass both a criminal background check and drug screen
