Job summary

PurposeCare is seeking a Care Transition Coordinator to grow our Home Health/Home Care presence in our service areas of Indiana. You will develop relationships with hospitals, case managers, and community resources to generate referrals and expand our footprint.

Responsibilities include managing the referral process, coordinating care, reporting on activities, and representing the agency professionally while complying with HIPAA and company policies.

Qualifications

• Bachelor’s Degree preferred.
• One year of Home Health/Home Care, marketing, or case management experience or a combination.
• Must possess a valid driver’s license.
• Uphold policies and procedures of Purpose Home Health; excellent oral, written, and active listening skills.
• Ability to work independently, maintain confidentiality, and meet deadlines.
• Demonstrate sound judgment and good decision-making skills.
• Establish good working relationships internally and externally.
• Ability to sit and listen effectively on the telephone.

The Care Transition Coordinator is an experienced field marketer responsible for company growth. As a Care Transition Coordinator , you will be responsible for developing our Home Health /Home Care presence within Supportive Livings and Home and Community-based residences. Responsible for generating new referrals by building relationships with area hospitals, healthcare systems , Case Managers, and other community resources.

Essential Duties and Responsibilities:

• Participate in short and long-range planning to achieve company goals and objectives.
• Responsible for complete referral process, including identifying home health needs, verification of benefits, and coordination of care.

Education and Experience :

Qualifications and Requirements:

• Minimum 18 years old
• Ensure the schedule is completed in such a way as to retain all clients, hours, and caregivers
• Communicate staffing and availability needs to the appropriate supervisor
• Positive attitude, great communication skills, and ability to work independently
• A solid personal character with an empathetic personality
• Must be able to pass both a criminal background check and drug screen
