Job summary

PurposeCare is seeking a Business Development Specialist - Home Care to expand our Home Health/Home Care referrals in Castleton, Indiana. This role focuses on building relationships with hospitals, healthcare systems, case managers, and community resources to grow our presence in Supportive Living and home-based settings.

The ideal candidate will have a marketing or case management background, strong communication skills, and the ability to work independently while meeting deadlines.

Qualifications

• Bachelor's degree preferred.
• 1 year of home health/home care marketing or case management experience.
• Must possess a valid driver's license.

Responsibilities

• Participate in short and long-range planning to achieve company goals.
• Manage complete referral process including identifying home health needs and coordinating care.
• Cultivate new and existing relationships to drive company growth.

About Us

Overview :

The Care Transition Coordinator is an experienced field marketer responsible for company growth. As a Care Transition Coordinator , you will be responsible for developing our Home Health /Home Care presence within Supportive Livings and Home and Community-based residences. Responsible for generating new referrals by building relationships with area hospitals, healthcare systems , Case Managers, and other community resources.

Essential Duties and Responsibilities:

• Participate in short and long-range planning to achieve company goals and objectives.
• Responsible for complete referral process, including identifying home health needs, verification of benefits, and coordination of care.
• Cultivate new and existing relationships resulting in company growth.
• Represent the agency in a professional, prepared, and caring manner.
• Prepare weekly reports on all marketing activities.
• Initiate and expand referral network.
• Comply with all aspects of the company's compliance and HIPPA regulations.
• Performs other duties as assigned.

Education and Experience :

• Bachelor's Degree Preferred
• One year of H ome H ealth /Home Care , marketing, or case management experience or a combination of each.
• Must possess a valid driver's license.
• This professional will uphold the policies and procedures established by Purpose Home Health. They must also demonstrate excellent oral, written, and active listening skills.
• Ability to work independently, maintain confidentiality, and meet assigned deadlines.
• Demonstrate sound judgment and good decision-making skills.
• Uphold integrity and fair-mindedness consistent with company standards, practices, policies, and procedures.
• Ability to organize and prioritize work.
