Centerstone in Nashville, TN is seeking a part-time Behavioral Health Aide to support youth and families by developing life skills and personal goals. You will work with a multidisciplinary team to coordinate care, document progress, and connect clients to community resources.

Formación

• Associate degree required; two years of experience or two years of college coursework in related field may be substituted for degree requirement.
• Two years professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.
• Experience in child welfare, school, childcare, or behavioral health settings is advantageous.

Responsabilidades

• Coordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including treatment plans and progress notes.
• Provides crisis intervention/prevention to clients and their families and connects clients with community resources.
• Maintains communications with schools, family service providers, and other partners involved in the client’s care.
• Delivers services according to organizational guidelines and protocols and ensures documentation compliance.
