Job summary

SYNERGY HomeCare, serving Middle Tennessee, seeks a Care Relations Coordinator for our Murfreesboro and Franklin offices. This full-time, office-based role focuses on connecting families with caregivers, guiding consultations, and onboarding new care professionals to ensure seamless care delivery.

The ideal candidate brings hands-on ADL experience, strong relationship skills, and a customer-service background. High school diploma required; CNA/MA cert preferred.

Qualifications

• Hands-on experience with activities of daily living (ADL) or personal care.
• Strong relationship and communication skills.
• Office and customer service background; comfortable with technology and coordination.
• CNA and/or Medical Assistant certification preferred (not required).
• High school diploma required; some college preferred.

Responsibilities

• Answer inquiries from families and find the right path forward for each one.
• Conduct care consultations and build initial plans of care for new clients.
• Introduce care professionals to clients and bridge the relationship from day one.
• Interview and onboard new care professionals, setting them up to succeed.
• Maintain consistent communication with clients, families, and care professionals.
• Build relationships with local healthcare organizations and referral partners.
• Partner with scheduling to ensure seamless care delivery.
• Review care notes and flag issues.
• Occasionally provide direct backup care in the field.
