ComForCare Home Care - Clarksville in Hendersonville, TN is seeking a compassionate and organized Care Coordinator to join our growing team. You’ll be the vital link between clients, caregivers, and families, ensuring each person receives high-quality in-home care.

This role is part-time, with 50% in the office and 50% in the field with clients. You will coordinate schedules, assess needs, support caregivers, and maintain accurate records to uphold our service standards.

Qualifications

• Experience in home care, healthcare coordination, or related field preferred.
• Strong organizational and multitasking skills in a fast-paced environment.
• Excellent communication and interpersonal skills with clients, families, and staff.
• Proficiency with scheduling software and basic computer applications.
• Ability to problem-solve quickly and remain calm under pressure.
• Compassionate, patient-centered mindset with a commitment to quality care.
• High school diploma or equivalent; associate's/bachelor's in health-related field a plus.

Responsibilities

• Coordinate and schedule caregivers to match client needs, preferences, and care plans.
• Serve as the primary point of contact for clients, families, and caregivers regarding care services.
• Conduct client assessments and develop individualized care plans in collaboration with the care team.
• Monitor caregiver performance and ensure compliance with home care standards.
• Manage caregiver scheduling, shift changes, and on-call coverage to maintain continuity of care.
• Maintain accurate client and caregiver records in compliance with regulatory requirements.
• Assist with caregiver onboarding, training, and ongoing support.
• Respond promptly to client concerns and resolve service issues with professionalism and empathy.
