Job summary

SYNERGY HomeCare — Middle Tennessee is seeking a Care Relations Coordinator for Murfreesboro and Franklin. This full-time, office-based role focuses on building trust with families, coordinating care, and guiding clients from consultation through onboarding.

You’ll answer inquiries, conduct care consultations, introduce caregivers, and stay in touch with families and professionals. Base salary is $40,000 with total compensation of $45,000–$55,000, plus paid holidays and PTO.

Qualifications

• Experience with ADL or personal care; understands caregiver/client needs.
• Strong relationship and communication skills; builds trust quickly.
• Office and customer service background; comfortable with technology.
• CNA or Medical Assistant certification preferred but not required.
• High school diploma; some college preferred.
• Ability to commute to Murfreesboro and Franklin offices.

Responsibilities

• Answer inquiries from families and identify the best care path.
• Conduct care consultations and draft initial plans of care.
• Introduce care professionals to clients to start the relationship.
• Interview and onboard new care professionals for success.
• Maintain ongoing communication with clients, families, and staff.
• Build relationships with local healthcare organizations and partners.
• Collaborate with scheduling to ensure seamless care delivery.
• Review care notes and flag issues needing attention.
• Occasionally provide direct backup care in the field when needed.
