Job summary

Merakey is seeking a Program Specialist to join our team in Westmoreland County, PA. This role manages day-to-day client care within the identified division, coordinating activities and ensuring high-quality services to program participants.

You will handle service plan development, collaborate with Nursing and Supports Coordinators, and work with Behavior Specialists to monitor and adjust plans. A Bachelor's degree and 3–5 years' experience are preferred; strong communication and teamwork are

Qualifications

• Bachelor's degree required.
• Total years of Experience: 3 to 5 years.
• Excellent interpersonal, communication, and conflict resolution skills.

Responsibilities

• Overall coordination of individual care and services.
• Client Care and Safety.
• Responsible for service plan development and implementation.
• Collaborates with Nursing to address Consumer healthcare issues, prescribed medications, discharge instructions, Consumer-specific adaptive equipment needs, and delivery of Individual-specific training.
• Meets with Supports Coordinators (SC)/Case Manager to transfer ISP data.
• Collaborate with Behavior Specialist regarding Behavior Support Plan; submits to Peer Review; works with management regarding response and plan of action; monitors adherence to plan.
• Perform other duties as assigned
