Job summary

American Medical Administrators LTC is seeking a Care Plan Coordinator to lead patient care planning in a long-term care setting. You will assess patient needs, develop individualized plans with interdisciplinary teams, and ensure compliance with Medicare guidelines and HIPAA.

You will also perform chart reviews, support nursing staff, and maintain accurate documentation in the electronic health records system while prioritizing quality and patient outcomes.

Qualifications

• Requires an unencumbered RN or LPN license.
• 2–5 years of experience in long-term care.
• Knowledge of Medicare regulations and requirements.

• Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.

Requirements:

• Unencumbered RN or LPN license

• Knowledge of Medicare regulations
• Commitment to Total Quality Management
• Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing
