Job summary

American Medical Administrators LTC is seeking a dedicated Care Plan Coordinator to join our long-term care team. The role focuses on assessing patient needs, developing individualized care plans, and ensuring compliance with Medicare guidelines and HIPAA.

You will collaborate with interdisciplinary teams, perform utilization reviews, and support nursing staff with documentation practices to optimize patient care and outcomes.

Qualifications

• Hold an unencumbered RN or LPN license.
• 2–5 years of experience in long-term care.
• Knowledge of Medicare regulations.
• Commitment to Total Quality Management.
• Working knowledge of Case Mix Index, Quality Measures tracking, QRP, and Value Based Purchasing.
