CARE PARTNERS, Timberwood - Albany, OR is seeking a Care Partner to support elders with daily living activities and holistic well-being. This role emphasizes compassionate care, communication with residents and families, and collaboration with the Wellness Team.

You will contribute to a vibrant, elder-centered community, assisting with daily routines, health monitoring, and engaging residents in meaningful activities in a supportive environment.

Formación

• Ability to provide personal care to elders.
• Ability to observe and report health changes to the Wellness Team.
• Willingness to work with a team and follow care plans.
• Pass a criminal background check as required by state law.

Responsabilidades

• Provide personal care: bathing, dressing, grooming, toileting, mobility assistance.
• Monitor elder health and document changes for the Wellness Team.
• Provide emotional support and companionship to residents.
• Assist with meals and help with feeding as needed.
• Engage residents in social and recreational activities.
• Communicate with residents, families, and other team members.

What You'll do

As a Care Partner, you can expect a range of responsibilities and tasks. Here are some general expectations:

• You will provide personal care: assist elders with daily living activities such as bathing, dressing, grooming, toileting, and mobility, possibly with walkers and wheelchairs.
• You will provide health monitoring and documentation: observe, document, and report changes in the elder’s health or behavior to the Wellness Team.
• You will give emotional support: may support elders dealing with loneliness, anxiety, or other emotional challenges. Engaging in conversation, active listening, and offering companionship is part of your role.
• You will assist with Meals: help elders with meals and possibly feed elders who require additional support.
• You will engage in social and recreational activities: supporting elders in life-enriching activities for their well-being.
• You will connect: Communicate with elders, their families, other team members, and guests.
• You will keep things clean: may involve light housekeeping, such as tidying up and ensuring general cleanliness.

What You'll Bring

• You will bring a willingness to learn and work within a team environment.
• You will bring good verbal communication and the ability to read, write, and speak English with residents and coworkers who only speak English.
• You will bring licensing documentation (if required by your state) and be able to pass a criminal background check.
• You will bring the ability to perform job responsibilities with or without accommodation.
