Job summary

Appone in Silver Creek - Woodburn, OR is seeking Care Partners to provide personal care to seniors, assist with daily living activities, monitor health, and offer companionship. The role includes Swing, NOC, Day, and On-Call shifts.

You will collaborate with the Wellness Team to document changes, ensure safety, and help with meals and light housekeeping. English communication is essential; licensing and background checks may be required.

Qualifications

• Willingness to learn and work within a team
• Good verbal communication and ability to read, write, and speak English with residents and coworkers
• Licensing documentation (if required by your state) and be able to pass a criminal background check
• Ability to perform job responsibilities with or without accommodation

Responsibilities

• Provide personal care: assist elders with daily living activities (bathing, dressing, grooming, toileting, mobility)
• Observe, document, and report changes in health or behavior to Wellness Team
• Offer emotional support and companionship to residents
• Assist with meals and feeding if needed
• Engage in social and recreational activities for wellbeing
• Communicate with elders, families, team members, and guests
• Maintain cleanliness with light housekeeping

What You’ll do

As a Care Partner, you can expect a range of responsibilities and tasks. Here are some general expectations:

• You will provide personal care: assist elders with daily living activities such as bathing, dressing, grooming, toileting, and mobility, possibly with walkers and wheelchairs.
• You will provide health monitoring and documentation: observe, document, and report changes in the elder’s health or behavior to the Wellness Team.
• You will give emotional support: may support elders dealing with loneliness, anxiety, or other emotional challenges. Engaging in conversation, active listening, and offering companionship is part of your role.
• You will assist with Meals: help elders with meals and possibly feed elders who require additional support.
• You will engage in social and recreational activities: supporting elders in life-enriching activities for their well-being.
• You will connect: Communicate with elders, their families, other team members, and guests.
• You will keep things clean: may involve light housekeeping, such as tidying up and ensuring general cleanliness.
