Job summary

OU Health in Oklahoma is seeking a Care Management Outcome Specialist to lead care coordination efforts within System Care Management. You will coordinate and evaluate patient outcomes, develop staff training, and collaborate with interdisciplinary teams to ensure safe discharge planning and high-quality care.

The role emphasizes performance improvement, regulatory compliance, and mentorship of staff through the hospital’s Preceptor Program. Strong communication and leadership are required.

Qualifications

• RN or Social Work licensure is required or under supervision.
• Minimum 5 years of care management experience, including acute or ambulatory care.
• Current Life Support (BLS) certification required.
• Certification in Case Management (CCM) or Healthcare Quality (CPHQ) highly preferred.

Responsibilities

• Plan, implement, and evaluate care coordination programs.
• Lead quality and outcome improvement initiatives.
• Develop and deliver staff training on evidence-based care.
• Coordinate discharge planning and continuum of care.
• Analyze patient data to measure program impact.
• Collaborate with interdisciplinary teams and regulators.
• Ensure compliance with Joint Commission and CMS standards.
• Lead preceptor program and mentor staff.
• Perform other duties as assigned.
