Job summary

OU Health is seeking a Care Management Outcome Specialist in the System Care Management department. You will coordinate, evaluate and improve patient care outcomes while leading staff training on evidence-based care and discharge planning.

You will collaborate with interdisciplinary teams, analyze data, and ensure regulatory compliance to drive safe, timely, and high-quality care across the continuum.

Qualifications

• Minimum of 5 years in Care Management, including 1 year in acute or ambulatory care.

Responsibilities

• Planning, Implementation, and Evaluation of Care Coordination Programs to improve health outcomes.
• Quality and Outcome Improvement Initiatives: coordinate ongoing projects and assess effectiveness of care activities.
• Staff Development & Training to ensure competency in evidence-based care and discharge planning.
• Patient Discharge and Continuum of Care Planning with interdisciplinary teams.
• Outcome Evaluation & Data Analysis using clinical data and metrics to measure program success.
• Collaboration with nursing, medical staff, social work and other departments.
• Regulatory Compliance: ensure adherence to Joint Commission and CMS standards.
• Preceptor Program: develop and mentor new staff within the hospital.
