Job summary

Molina Healthcare seeks a care coordination professional to support LTSS and care management with members across the continuum. The position requires experience in health care or LTSS, licensure if required by state contracts, and strong communication skills.

Qualifications

• At least 2 years of health care experience, including LTSS
• 1 year of care management or medical/behavioral health setting experience
• Valid and unrestricted driver's license and reliable transportation
• Must be able to travel 25-40% locally
• Knowledge of community resources
• Ability to work independently with minimal supervision
• Strong verbal and written communication skills
• Proficiency navigating online portals and databases
• Experience with populations receiving waiver services (preferred)

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person home visits
• Facilitate waiver enrollment and disenrollment processes
• Develop and implement care plans with members and care team
• Monitor care plan effectiveness and document progress
• Promote integration of LTSS and behavioral health services
• Assess medical necessity and authorize waiver services
• Facilitate ICT meetings for service decisions
• Identify barriers to care and coordinate resources
• Travel 25-40% as required by state/contractual requirements
