Qualifications

  • High School diploma or equivalent; Medical Assisting program preferred.
  • Knowledge of the day-to-day operations of a medical practice and basic patient care.
  • Excellent communication skills required both verbal and written.
  • The desire to grow professionally and to gain more responsibility as necessary.
  • Ability to handle complex workload that requires meeting daily deadlines.
  • Associates degree (preferred)
  • 2+ years of experience in a clinical informatics position or similar role (preferred)
  • 2+ years of clinical background such as CNA or MA (preferred)
  • Proficient in Microsoft Windows, Microsoft Office
  • Ability to learn multiple EMR Systems

Responsibilities

  • Call patients within 48 business hours of discharge from Hospital to schedule TCM and coordinate care.
  • Call patients within 48 business hours of discharge from an emergency department visit.
  • Conduct patient assessments including PHQ-9 and SDOH.
  • Attend and actively participate in Care Management related training and meeting activities.
  • Proactively acts as a patient advocate.
  • Adheres to HIPAA guidelines and regulations.
  • Recognizes and responds to opportunities for improvement based on established, evidence-based practice.
  • Provides mentoring/coaching of other population health and care coordination team members.
  • Cultivates effective partnerships, collaborates with all practice providers.
  • Effective interaction with ACO/PCMH coordination and integration of ACO/PCMH services.
  • Provide a coordinated, strategic approach to effectively manage the chronically ill patient population.
  • Coach patients/families toward successful self-management of their chronic disease.
  • Develop, monitor, and facilitate a care plan based on mutual goals with the patient, family, and provider's plan.
  • Facilitate patient access to appropriate medical and specialty providers.
  • Serve as the contact-point, advocate, and informational resource for patient and care team.
  • Facilitate and attend meetings between patient, families, care team, payers, and community resources.
  • Set expectations through measurable goals and objectives and monitor progress.
  • Train staff in electronic health record usage.
  • Monitor compliance with state requirements/guidance.
  • Participate in quality improvement projects and trainings.
  • Maintain awareness of quality and safety measures.
