Whitman-Walker Health in Washington, DC is seeking a Care Navigator to support adults with HIV or treatment adherence challenges. You will meet patients face to face, virtually, or by phone to educate, counsel, and connect them with Whitman-Walker services and community resources.

Work as part of a multidisciplinary team, document activities in EMR, and coordinate referrals with clinicians and community partners to improve retention in care.

Qualifications

• Two or more years of paid work experience providing direct assistance in public health or healthcare settings.
• Bachelor's degree in psychology, social work, public health, or related field.
• Experience with HIV outreach, linkage and retention preferred.
• Experience working with diverse populations.

Responsabilités

• Provide specialty adherence care and coordinate with the care team.
• Engage out-of-care patients and refer to community services as needed.
• Develop individualized care plans to promote adherence and retention.
• Maintain up-to-date records in EMR and prepare reports.
