Whitman-Walker Health in Washington, DC is seeking a Medical Adherence Nurse Care Manager to join a multidisciplinary team. The RN will provide primary and specialty care emphasizing medical adherence, self-management, health promotion, and harm reduction.

Develops care plans and coordinates referrals to internal or external resources. The role requires knowledge of HIV-related conditions, strong communication, and ability to work across diverse patient populations.

Qualifications

• BSN required; DC RN licensure active.
• MSN or MPH preferred.
• 1–3 years post-RN licensure preferred.
• Experience with HIV care preferred.

Responsabilités

• Provide primary and specialty care focusing on adherence and disease management.
• Develop care plans and refer to internal/external resources.
• Monitor medical progress and HIPAA compliance.
• Collaborate with multidisciplinary team to optimize care.
