Job summary

Whitman-Walker Health seeks a Cancer Patient Navigator to serve as the primary contact for participants in Whitman-Walker’s Cancer Navigation Program. You will provide culturally responsive, trauma-informed navigation across the cancer care continuum and collaborate with clinical teams, oncology partners, and community organizations to reduce barriers to care and improve access.

Qualifications

• Bachelor’s degree in Public Health, Social Work, Human Services, Nursing, or related field preferred; equivalent lived and professional experience considered.
• Minimum 2–3 years of experience in cancer navigation, oncology services, patient navigation, case management, or chronic disease management required.

Responsibilities

• Serve as the primary contact for all Cancer Navigation Program participants.
• Conduct comprehensive needs assessments to identify barriers to cancer care and survivorship.
• Provide patient education on cancer prevention, screening, treatment options, and follow-up care.
• Coordinate insurance enrollment, benefits navigation, and financial assistance resources.
• Connect patients to transportation, childcare, housing, food access, and other supportive services.
• Document interactions in the electronic health record and tracking systems.
