HealthFirst is seeking a Care Coordinator to support NorthStar Members by assisting with navigation of the health care system, handling member calls, and coordinating with care managers across teams. The role requires excellent communication, organizational skills, and the ability to manage multiple priorities in a fast-paced environment, with careful documentation of interactions in the PHI system.

You will contribute to delivering high-quality customer service and support member outcomes

Qualifikationen

• CMA, CNA, or RMA certification or relevant medical training.
• HS diploma or GED required.
• Excellent verbal and written communication with strong grammar and punctuation.
• Experience in a fast-paced environment managing multiple priorities.

Aufgaben

• Receive incoming calls from members regarding benefits, care coordination, and program inquiries.
• Document calls contemporaneously in designated PHI system.
• Manage a caseload requiring short-term care coordination and health navigation.
• Collaborate with the interdisciplinary care team to address member needs and monitor outcomes.
• Perform outreach calls to confirm services are in place.
• Additional duties as assigned.
