Job summary

Anchor Co-Living in Sacramento is seeking a compassionate Care Coordinator who can support members through recovery, connect them to resources, and help them navigate housing opportunities.

You will document care plans, coordinate transportation, meals, and referrals, and ensure timely updates in the EMR. This role blends clinical knowledge with human connection to prevent gaps in care.

Qualifications

• LVN or higher clinical credential required.
• Experience in care coordination, case management, or community-based services.
• Strong written and verbal communication skills.
• Strong, timely, and accurate documentation habits.
• Experience working in EMR systems.
• Ability to coordinate across multiple providers, organizations, and priorities.
• Strong follow-through and attention to detail.

Responsibilities

• Coordinate daily member check-ins and document in the EMR.
• Coordinate meals and other essential needs.
• Communicate with healthcare providers and community partners.
• Facilitate referrals to services and resources.
• Help members arrange transportation to appointments.
• Track authorizations and renew before they lapse.
• Support MCP reporting and related requirements.
• Ensure documentation is complete and accurate.
