Job summary

Regard Management PA LLC seeks a dedicated Care Manager to advocate for patients, coordinate services, and support recovery from opioid use disorder. The role emphasizes collaboration with diverse providers, community partners, and internal teams to ensure holistic care.

You will perform assessments, document progress, maintain confidentiality, and build rapport with clients from varied backgrounds while traveling between offices as needed.

Qualifications

• Bachelor’s degree with 18 college credits in sociology, social welfare, psychology, criminal justice, or related social sciences with personal recovery experience.
• Or an Associate’s degree in a social science field with any equivalent combination of experience and training with lived recovery experience.
• Ability to interpret regulations, policy and procedures and apply them accordingly.

Responsibilities

• Advocates for patients and helps coordinate access to health and support services.
• Develops partnerships within the community for patient-centered care.
• Assists in coordinating access to addiction treatment and related services (housing, childcare, medical and financial services).
• Maintains effective working relationships with staff, external agencies, and the public.
• Documents patient care services and maintains confidentiality and privacy.
