Job summary

PCC Community Wellness Center is seeking an RN for the Recovery Nurse Care Manager role in Chicago. The position focuses on coordinating screening and intervention for patients with complex chronic care needs, including substance use recovery, acting as a bridge between patient needs and ARC resources.

The RN collaborates with a multidisciplinary team to support whole-health outcomes, advocate for patients with providers and payers, and ensure access to necessary medications and services.

Qualifications

• Graduate of an accredited school of nursing.
• 2–3 years clinical experience preferred.
• 2–3 years EMR experience preferred.
• Strong communication with patients, families, and healthcare providers.
• Ability to manage multiple tasks in a fast-paced environment.
• Proficient in MS Office (Outlook, Word, Excel).

Responsibilities

• Coordinate screening and intervention for patients with complex chronic needs, including substance use recovery.
• Support ARC program coordination and care transitions.
• Provide therapeutic support in clinic, groups, and via phone.
• Collaborate with interdisciplinary team to ensure coordinated care.
• Maintain patient registry and complete outreach as needed.
• Assist with referrals to higher levels of care and payer coordination.
