Job summary

PCC Community Wellness Center is seeking a Recovery Nurse Care Manager (RN) to coordinate screening and interventions for patients with complex chronic needs, including substance use recovery, within the ARC program in the Chicago area.

The role will focus on connecting patients with resources, supporting whole-health outcomes, and communicating progress to the healthcare team to ensure optimal care delivery.

Qualifications

• Graduate of an accredited school of nursing.
• 2–3 years clinical experience preferred.
• 2–3 years EMR experience preferred.
• Must be able to exchange accurate information with patients, families and healthcare providers.
• Demonstrated ability to work collaboratively on a healthcare team and handle multiple tasks.
• Proficient in MS Office (Outlook, Word, PowerPoint, Excel).

Responsibilities

• Coordinate screening and intervention for patients with complex chronic care needs.
• Support ARC program by increasing patient self-management and facilitating referrals.
• Provide therapeutic support in clinic, groups, and by phone.
• Consult with residents and physicians in bio-psycho-social care coordination to improve health outcomes.
• Educate patients and families on disease management and treatment options.
• Maintain patient registry and outreach as needed.
