Job summary

Qualifications

• At least 2 years experience in health care, care management, or related setting.
• Clincial licensure or certification required only if state contract/mandate requires it.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources.

Responsibilities

• Completes member assessments per timelines to determine eligibility for care coordination/management.
• Develops and implements care plans with member, caregiver, and health professionals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Maintains ongoing member caseload with regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
